Registration paperwork
To save time, please complete your forms before your visit. You can fill them out online or download and bring them with you. Don’t forget a photo ID — there’s no time set aside for paperwork during your appointment.
Insurance & Financing
Dental insurance can be complicated — we’ll help you understand your coverage, what you’ll owe, and handle coordination with your provider. We accept most major PPO plans and offer complimentary benefits checks.
No insurance? We offer flexible payment plans, interest-free financing through CareCredit and Lending Club, and our Care+ Membership for affordable monthly wellness visits. Our team will help you find the best option for your needs and budget.
X-Rays & Records
If you’d like your previous dentist to send X-rays or records, they can email them to us before your visit. No worries if you don’t have them. We’re happy to take new x-rays at your appointment. Past records can be helpful but aren’t required for care.

What to expect at your first visit
Whether it’s a dental wellness visit or an emergency appointment, your first visit is thoughtfully scheduled to ensure there’s plenty of time to get to know you and provide the care you need.
Dental wellness visits (90 minutes) include X-rays, an oral cancer screening, cleaning and polishing, a comprehensive exam, and time to discuss your oral health goals.
Emergency visits (60 minutes) focus on diagnosis and fast relief, with the option to start treatment the same day often available.
In both cases, we’ll clearly explain any recommended next steps, and our financial coordinators are here to walk you through fees and flexible payment options.

As a Central Park West Dentistry patient, you’ll have access to the latest technology and advancements in dentistry. We have everything from low radiation digital x-rays and laser-assisted periodontal surgery, to 3D scanners and intraoral cameras that let you see what your dentist sees.

Your account
We’re happy to handle insurance claims on your behalf — we’re paperwork pros! While we do our best to estimate your portion based on the information your insurance provides, any remaining balance is your responsibility. Need flexibility? Payment plans are available, and you can easily pay by phone with a quick call or through our patient portal online.
