You can also download the forms here, print them, complete them at your leisure, and bring them with you to your 1st visit. There’s no time for paperwork built into your visit, so it’s essential you fill this out in advance to help ensure you have the full reserved time available for your care. Please also bring a photo ID with you on the day of your appointment.

We accept most major PPO dental plans. Exactly what’s covered depends on the specific details of your plan. Please give us a quick call or send us an email and we’ll be happy to do a complimentary benefits check for you to let you know how your benefits will work next time.

We understand that investing in quality dentistry can put a strain on your budget, so we offer flexible financing options and customized payment plans to help.
We accept all major credit cards and cash payments. Payment arrangements and interest-free financing are available through The Lending Club and Care Credit. Our financial coordinators will work with you to determine the very best financial arrangement for you, based on your budget, credit history, and treatment plan.
For people without insurance who would like to make manageable monthly payments towards their dental wellness visits, we also offer a Hygiene Membership.
If you’d like your previous dentist to send x-rays or records to our office before your visit, they can send them via email. If you haven’t been to the dentist for a long time or don’t have access to your records, don’t fret!
We can easily retake any x-rays for you at the time of your appointment.
In some cases, having past records can help our providers gain an understanding of a complex dental history or avoid duplicate x-rays – but it is by no means necessary for care.
